Dear Interpreters,

Sign Language Resources, Inc. (SLR) will need the following lists of documents on file prior to any assignments SLR can request of you. These documents are required of us by our various contractors and the IRS. All information and documentation is kept confidential. Once established, you will be asked to periodically update these documents, or you can update them at anytime there is a change. These documents are:

1) proof of car insurance
            (copy of insurance card)

2) proof of disability insurance - if eligible
           
(copy of paid premium)
        
-OR-
            signed, original copy of the Hold Harmless Agreement

3) proof of malpractice insurance - if certified
           
(copy of paid premium)
        
-OR-
            signed, original copy of the Hold Harmless Agreement
         
    (click here)

4) proof of RID certification - if applicable
           
(2 photocopies of RID card)

5) a current resume

6) W-9 form (click here)

7) Signature page of the "SLR Policies & Information Packet"

8) If working on IBM assignments, a CDA
         
  Confidential Disclosure Agreement

Please note: If you are not yet certified, please also supply us with the following 2 referrals:
  1 from an interpreter colleague whom you have worked with;

  1 from a deaf consumer you know and/or have worked with.
